Keywords:Home care is care and nurture given in the home of the client. It includes home services and at-home hospital care.
You will be given home care based on the individual care and rehabilitation plan. As a client of home care, you can live at the comfort of your own home for as long as possible. Your resources, need of care, functional ability and sense of safety will be taken into account in your care.
Home care will help you cope with daily activities. Your independence will be maintained with daily rehabilitation. Your service need will be regularly assessed with you and the services will be organised according to your changing situation. In addition to home care, it is possible to supplement your services with different support services. They include, for example, safety service, meal service, washing service, errand service, cleaning service, transport service and daytime activities service.
The wellbeing services county of Southwest Finland offers this service starting from 1 January 2023.

The service can also be provided with a service voucher.
Requirements
You are eligible for home care if:
- Your functional capacity is clearly lowered and you cannot cope, with the help of your relatives or next of kin, with regular daily basic activities, such as eating, getting dressed, using the toilet, washing, other basic care or medicinal care. We will assess your service need by estimating your functional capacity and the illnesses that hinder coping at home, your living conditions and social network.
- Due to illnesses or lowered functional capacity, you require special professional care, examinations and health monitoring and you cannot use health services outside the home.
- Your relative is caring for you and requires the support of home care.
The service is liable to charges.
User instruction
Contact the service counselling for older persons or the home care of your municipality of residence. The home care will be adapted according to your needs.
The service can also be arranged with a service voucher.
If you are a new customer, you can contact the elderly customer service centre. If you are already a home care customer, you will be given contact details by your own home care team when the customer relationship begins.
