Contact your own school's public health nurse in the first instance, if necessary, you can contact the Student Health Care centralised helpline or chat.

The pre-information form for student health care is intended for students in upper secondary vocational education or upper secondary school. The form is used to collect preliminary information for a health check by a public health nurse. The student fills out the form themselves before the health check. Please note that if you have filled in and returned the paper form, you do not need to fill out the electronic form again. More information of the health check and the form is available from the health nurse of your own educational institution. Please note that the forms will not be processed in July.
